Patient information: Male, 23 years old
Preoperative diagnosis: skeletal class III malocclusion
Surgical plan: Maxillary LeFort I osteotomy followed by bilateral distraction osteogenesis and mandibular sagittal split osteotomy
After two years of orthodontic decompensation treatment, the patient was evaluated for simple LeFort osteotomy. Sagittal split osteotomy of the mandible could not solve the problems of insufficient maxillary bone and huge overjet distance between the upper and lower jaws. At present, retractors are divided into intra-oral and extra-oral retractors. The huge shape of extra-oral retractors will not only leave scars on the face but also affect the patient's normal life. There is currently no suitable finished retractor in the domestic market for intra-oral retractors. The hospital's orthodontic and orthognathic team digitally designed and simulated the feasibility of the surgical plan for the patient, and carried out after-sales modification of the built-in distraction osteogen. Based on preoperative digital analysis and simulation, the patient underwent maxillary LeFort I osteotomy, bilateral maxillary distraction osteogenesis, and mandibular sagittal split osteotomy.

About the amazing "distraction osteogenesis" Distraction osteogenesis uses a specific device (bone traction device) to exert a continuous and slow traction force on the severed bone segment. This traction force will promote the simultaneous regeneration of bone tissue and surrounding soft tissue, thereby lengthening or widening the bone. This technology was first used in orthopedics, and was later gradually applied to maxillofacial surgery. For example, we can cut the maxillary bone and use different types of retractors to gradually move the maxillary bone in the required direction every day, slowly increasing the maxillary bone to improve the problem of maxillary retrusion.
